Answer to Question 1

A
Decreased vaginal secretions decrease the natural lubrication of the vagina, which can cause painful intercourse and increase the potential for vaginal infections. Responses B, C, and D are unrelated to painful intercourse. Urinary incontinence can increase the incidence of vaginal infections.

Answer to Question 2

C
The LPN can do an adequate visual inspection looking for broken teeth, inflammation, lesions, pallor, drainage, or coating on the tongue. Visual inspection of the mucous membranes is also indicated. A smooth, painful tongue may indicate vitamin B12 deficiency. Follow-up with a dentist may be recommended based on the nurse's findings.
