No awareness, denial, vague awareness, preplanning, preparation, initiation, stabilization, confirmation/expansion, professionalism

Answer to Question 2

Maintenance refers to subjects having maintained their changed behavior for 6 months.
Termination refers to subjects having no chance of returning to the old behavior.
