The security provisions should include the following policies and procedures:
 Define authorized users of patient information to control access

 Implementation of a tracking procedure to sign out records to authorized personnel

 Limit of record storage access to authorized users

 Locked record storage areas at all times

 Requiring the original medical record to remain in the facility at all times

Answer to Question 2

The proposed standard for electronic signatures is digital. Encrypt means to encode a computer file, making it safe for electronic transmission so that unauthorized parties cannot read it. Decrypt means to decode a computer file.
