Any three of the following:
Establish time frames for reporting and evaluation of the occurrences.
Establish reporting of occurrences as a positive means of improving the delivery of quality care as opposed to a punitive performance evaluation.
Record only the facts.
Narrative description should be written in the first person and recorded by the employee who witnessed or experienced the occurrence.
Incident reports should not be maintained with the medical record; however, the chart should be documented regarding the occurrence, reflecting the continuation of the treatment provided to the patient.
Categorize occurrences and utilize reports to identify trends and patterns.
Share the reports with the appropriate committees, such as the quality improvement committee, to improve processes that are risks and potential risks to the company.
