Coders can code only what is documented in the patient's health record. Poor clinical documentation by the physician hinders correct code assignment by the coders. Codes equate to reimbursement for services rendered to the patient. If the clinical documentation is not adequate, the coding may not be accurate, which results in lower revenue for the health care entity or physician.
