The hospice organization is accredited by The Joint Commission; therefore, CMS allows The Joint Commission to survey the hospice organization and deem the hospice as meeting the
certification requirements for Medicare/Medicaid.

Answer to Question 2

The seven minimum parts are:
(1) initial and comprehensive assessments
(2) plan of care, including interdisciplinary group updates
(3) signed copies of patient rights, hospice election, and informed consent
(4) responses to medications and treatments, with related physician orders
(5) outcome measure data
(6) physician certification and recertification of terminal illness
(7) copies of advance directives
