One advantage of source-oriented records is that they organize reports from each source together, making it easy to determine the assessment, treatments, and observations that a discipline has provided. A disadvantage is that it is not possible to quickly determine all of the patient's problems.

An advantage of problem-oriented records is that the individual directing the patient care is required to consider all of the patient's problems in total context. A disadvantage is that the format usually requires training of the professional staff.

Answer to Question 2

A source-oriented record is the traditional way in which a record is organized in sections according to patient care departments and/or disciplines. Within each section the forms are arranged according to date. A problem-oriented record provides a systematic method of documentation to reflect logical thinking on the part of the person directing the care of the patient. The individual directing the care of the patient defines and follows each clinical problem individually and organizes the problems for solution.
