Answer to Question 1

The medical home involves the primary care provider working with a team of health care professionals to provide care to patients using a whole-person concept. The primary care professional either provides all of the care to the patient or arranges for other care regardless of the site of care or the patient's stage of life.

Answer to Question 2

(1) Continuing shift of care to the ambulatory setting and (2) increased integration of freestanding ambulatory care sites with other health care facilities.
