Answer to Question 1

Any two of the following:

Providing patients with timely electronic access to their health information within 96 hours of the information being available to the provider.

Providing patients with an electronic copy of their health information, upon request. Providing inpatients with an electronic copy of their discharge instructions and procedures at time of discharge, upon request.
Sending reminders to patients, per patient preference, for preventive and follow--up care.
