Do writers always state a cause-and-effect relationship directly? How else
  might the relationship be indicated? How does the reader determine the
  relationship when it is not directly stated?

A definition is an explanation of the meaning of a word or term.

Answer to Question 2

Writers do not always state a cause-and-effect relationship. The
relationship may only be implied. The reader must determine the
relationship by making an inference.
