You are caring for a patient with an ileal conduit. You notice there is tape around the drain pouch. When you ask the patient about it, he tells you it was leaking and the previous PCT taped the area that was leaking. What actions should you take?

Answer to Question 1

Notify the nurse. The pouch needs to be changed. Pouches are never taped when leaking; a new pouch is applied. The nurse will also need to address proper ileal conduit teaching with the PCT that taped the leaking pouch.

This behavior is known as sundowning. Validation therapy, one-on-one care, reassurance, distraction, and recollection of past memories from a memory box are several helpful strategies.
