You are caring for a patient with diabetes. You notice the person is forcing his shoes on over very swollen feet and the shoes are extremely tight. He denies any discomfort. Is this a problem?

Question 2

You are caring for a patient that has had surgery and now has an order for elastic stockings. The patient was alert and oriented before surgery but is a little confused at this time. She keeps rolling her stockings down and twisting them. Is this an issue?

Answer to Question 1

You are aware diabetics often have reduced sensation in the feet. Shoes that do not fit properly can cause wounds and because the patient is a diabetic he is at greater risk for infection and other complications. Remove the shoes. Notify the nurse that the patient's feet are swollen, as this too needs to be addressed.

Answer to Question 2

Creases, wrinkles, and twists can impair circulation. When the stockings are rolled down they are not effective. Assure circulation is not impeded, the patient is safe, and notify the nurse of the issue.
