The nurse should assess for elevated blood pressure because hypertension is associated with the development of AD. A series of small strokes is seen in clients with multi-infarct dementia, not AD. Abuse of cocaine, particularly crack cocaine, is associated with crack-related dementia. A low level of the vitamin folate is a risk factor for the development of AD.
