Answer to Question 1

D
Feedback:
Vertex presentation occurs when the fetal head is flexed well against the fetal chest and the top of the fetal head is the presenting part. In normal labor, there is a vertex presentation. Complicated labor often occurs when body parts other than the top (crown) of the fetal head present. When the buttocks, foot, or knee is the presenting (lowest) part, it is called a breech presentation. The shoulder may be the presenting part if the fetus is lying in a transverse position. If the head is partially extended, then the brow presents.

Answer to Question 2

D
Feedback:
A blunt injury to the eye may cause swelling and bleeding into the soft tissues surrounding the orbit, resulting in a contusion or hematoma, also known as a black eye. The nurse should apply cold packs for the first 24 to 48 hours to decrease bleeding and edema. When the swelling has stopped, usually after 24 to 48 hours, the nurse should use warm packs on the site to hasten the absorption of the blood from the tissues.

There can actually be a 25-hour time difference between certain locations in the world. The International Date Line passes between the islands of Samoa and American Samoa. It is not a straight line, but "zig-zags" around various island chains. Therefore, Samoa and nearby islands have one date, while American Samoa and nearby islands are one day behind. Daylight saving time is used in some islands, but not in others—further shifting the hours out of sync with natural time.
