The nurse should explain the surgery clearly and have the client sign the operative permit. In many cases, this will be done by the RN or surgeon. The nurse is responsible for making sure the permit is in the records. The nurse should also determine which of the client's regularly scheduled medications should be taken the morning of surgery and allow the client to take them. The nurse should make sure arrangements have been made for the person to get home after surgery because clients are not allowed to drive, and make sure the client has someone to stay with him or her at home for at least 2448 hours after surgery.

The nurse should monitor for signs and symptoms of dehydration in the client. Other symptoms of ketoacidosis are increases in pulse and respiratory rate and hypotension. Monitoring of respiratory depression, decrease in pulse rate, and hypertension is not necessary because these are not problems usually associated with ketoacidosis.
