Answer to Question 1

A

Answer to Question 2

B
Feedback:
The catheter balloon must be completely deflated when the retention catheter is removed, to prevent urethral trauma. A cut catheter can retract back into the urethra or bladder, so it should never be cut and removed. Other measures, such as positioning the client in the side-lying position or removing the plastic cover of the catheter, are unnecessary at this stage. The side-lying position is generally maintained for female catheterization. The client should not be asked to hold his breath. Instead, he should be asked to breathe slowly and deeply to facilitate removal.
