Answer to Question 1

A
Feedback:
The nurse should monitor the client for hyponatremia. Water retention in excess of salt retention may be caused by an excess production of antidiuretic hormone (ADH) and may result in hyponatremia. Hypercalcemia, hypochloremia, and hyperphosphatemia are less likely to occur.

Answer to Question 2

C
Feedback:
Obstruction near the glottis causes a hissing, crowing sound. Spasms and edema of the bronchi do not cause a hissing, crowing sound; they cause asthma, a condition that causes difficulty breathing. Air passing through secretions present in the air passages causes stertorous breathing, but does not cause a hissing, crowing sound. Partially blocked air passageway causes snoring, which is not a hissing, crowing sound. Snoring is common during sleep, when the person's tongue falls back as a result of relaxation.
