Answer to Question 1

Ans: B
Feedback:
When charting nursing care, the nurse should record concise, accurate findings and avoid personal opinion. The nurse should not provide a personal opinion about the client's odor, appearance and uncooperative manner. A better way to chart the information would be the following: Refuses staff to bathe him and stated he wants to be left alone. When charting about the client, it is implied it is about the client, so the word client may be omitted.

Answer to Question 2

Ans: C
Feedback:
The authoritarian leader dictates what will be done, when it will be done by, and how it will be done. The leader will check on the team members often and will make decisions affecting the whole team without their input. The leader is a laissez-faire or passive leader who assumes the team knows what to do and will get the job done and offers no direction for the team. The leader is a democratic leader who encourages the team to participate, consult, and collaborate producing better decisions. The leader is a transformational leader who motivates the whole team to work together through effective communication, which leads to team building and increases productivity.
