Answer to Question 1

Ans: A, B, C, D
Feedback:
The nurse needs to include statements made by the perpetrator verbatim. With a complete description of the injury and use of injury maps and with permission, the agency may include photos as forensic evidence for the physical abuse/battery that occurred. There is strict liability, injury to the victim present and, since the UAP intentionally assaulted (verbally abused) and battered (physically abused) the victim, this constitutes an intentional tort. There is no evidence of false imprisonment, restricting the client against his will physically or verbally.

Answer to Question 2

Ans: C
Feedback:
Failure to provide the standard of care expected of a nurse in the same or similar circumstance can result in a malpractice suit or disciplinary action. The nurse should follow the institution's current standards of care over ones taught in nursing school or ones published by the government because they may conflict with the institution's philosophy and mission or become outdated. The nurse should not omit parts of a standard of care because the action could result in harm to the client.
