The nurse needs to include statements made by the perpetrator verbatim. With a complete description of the injury and use of injury maps and with permission, the agency may include photos as forensic evidence for the physical abuse/battery that occurred. There is strict liability, injury to the victim present and, since the UAP intentionally assaulted (verbally abused) and battered (physically abused) the victim, this constitutes an intentional tort. There is no evidence of false imprisonment, restricting the client against his will physically or verbally.
