 the patient's mouth wasn't fully opened
  the chin was too flexed

the head was rotated

Rationale: The AP open mouth should demonstrate the relationship of C1 and 2 centered in the open mouth and between the mandibular rami.

Rationale: The AP axial c-spine will utilize a 40 SID.
