By permitting the patient to talk openly the nurse can gather data and generate a plan of care. Open-ended questions elicit more detailed information than those that ask for one-word answers. Direct questions should be used with caution and are not suitable for collecting objective data.

Put the patient at ease, maintain self-respect and dignity of the patient, and use language that the patient can understand. Instill confidence by having a neat appearance and positive attitude and listen attentively.
