A patient has been admitted to the hospital. He or she has been telling the neighbors that someone is bugging the telephone line and listening in to all of his or her calls and thought processes.
 
  The patient is convinced that someone is out to kill me and is refusing to remove his or her clothing and wear the hospital gown during admission. As the nurse doing this admission, you:
  A) Let the patient remain in his or her own clothing
  B) Get the orderly to remove the patient's clothing
  C) Get additional staff to help you remove the patient's clothing
  D) Ask the patient why he or she is being so uncooperative
