Answer to Question 1

B
Seclusion is used when less restrictive measures have failed to help the client maintain control. One
of its benefits is to reduce overwhelming environmental stimuli affecting an extremely distractible
individual.

Answer to Question 2

A
Physical needs must be met to preserve life. A client who is semistuporous must be fed by hand or
tube, toileted, given range-of-motion exercises, and so forth to preserve physiological integrity.
Safety needs rank second to physical needs. Higher level needs are of lesser concern.
