Answer to Question 1

1
Rationale: Sundowner syndrome, or sundowning, is understood as confused behavior when the environmental stimulation is low. It is seen in clients with delirium and dementia. The client can become increasingly agitated, disoriented, or even aggressive/paranoid or impulsive and emotional later in the day and at night.

Answer to Question 2

2
Rationale: The nurse must understand the pathology involved in the child's mental health problems and the child's life experiences, as they may contribute to the child's problems as well as strengths. Physiological health problems, artistic abilities, and previous hospitalizations are components of the child's life experiences. These must be considered in order to develop interventions that are appropriate to the child.
