Answer to Question 1

1
Rationale: Axis I provides information regarding major mental disorders, as well as developmental and learning disorders. Axis I does not provide information about support systems, chronic conditions, or indicate if the client is in need of immediate medical attention.

Answer to Question 2

3
Rationale: A stable work shift is essential to successful management of the bipolar disorder as irregular shift patterns could cause an exacerbation in the disorder. The client may not be able to maintain employment if there are exacerbations in the illness. Mood disorders often have a biologic rhythm component as evidenced by sleep pattern disturbances. It is prudent to maintain a consistent schedule to prevent the occurrence of mood changes as opposed to waiting for symptoms to occur.
