A client is admitted to a day hospital following an episode in which he purchased a gun to use while standing guard over his property to prevent a neighbor from erecting a boundary fence.

His wife describes him as distrustful of the motives of others and often interpreting others' mo-tives as threats. She mentions that one time he accused her of having an affair with a neighbor with whom she chatted occasionally. The care plan will list the priority outcome as Client will:
  1. admit his action was excessive based on the circumstance.
  2. write the neighbor a letter of apology.
  3. demonstrate trust in the nurse.
  4. identify positive role models.

Question 2

A client with borderline disorder tells the nurse, It's hard to figure out who I am. Sometimes I'm sexually attracted to women and sometimes to men. The nurse using Freudian concepts can analyze this as a developmental problem related to:

  1. Lack of separation-individuation
  2. Isolation of affect during latency
  3. Impaired development of sexual identity during the phallic stage
  4. Overdevelopment of latency stage traits related to control issues

Answer to Question 1

ANS: 3
The paranoid distrustful individual must develop trust in at least one staff member before other therapeutic interventions can succeed in bringing about behavioral change. Option 1 would be an outcome that will require a longer time to achieve. Option 2 might not be an identified outcome. Option 4 is not a relevant outcome for this client.

Answer to Question 2

ANS: 3
According to Freud, identifying one's sexual identity takes place during the phallic stage of de-velopment. When sexual identity is not clearly established, the individual may express confusion in sexual preference. The other options do not relate to information given in the scenario.
