A client admitted on an involuntary basis is being treated for an acute exacerbation of paranoid schizophrenia. He refuses his morning dose of fluphenazine (Prolixin). The most appropriate ini-tial nursing response would be:
 
  1. If you do not take your medication, you will be placed in seclusion.
  2. Involuntarily committed clients cannot refuse medications.
  3. Let's discuss why you do not want to take your medication.
  4. If you do not take your medication, I must give you an injection.

A hospitalized client who has schizoaffective disorder is being treated with lithium carbonate. He repeatedly states he is a prisoner and is being poisoned by the pills the staff is giving him. He states his tremors are proof of the poisoning.
