The easier you make it for audience members to fill out an evaluation form, the more likely you are to get it back. The two main factors that keep people from filling out evaluation forms are time and complexity. Therefore, you need to design a simple form that can be filled out quickly and still yield valuable information.

Answer to Question 2

The vast majority of audience members have no opinion about you or your topic until you step up to the lectern and start speaking. Regardless of what they write on their evaluations, remember that you prepared as best you could and that you had valid reasons for the decisions you made about your speech. So, take comments more or less seriously, but consider the source before you get upset or disheartened.
