The best response by the nurse is: 1. High RN-to-client ratios result in decreased length of stay..
   2. Medication errors decrease the cost of care..
   3. Promoting safety increases the cost of care..
   4. Use of computers increases the number of law suits..

Question 2

The nurse critically evaluates various models of grief used for terminally ill clients and their families. What should the nurse recognize when applying these models to individual cases?

Answer to Question 1

1. High RN-to-client ratios result in decreased length of stay..

Rationale:
Studies have shown that increasing the RN-to-client ratio can decrease overall cost because the length of stay is reduced with RN care as well as mortality rates. Medication errors increase cost due to harm to the client and increased length of stay. Promoting safety is a cost-containment measure because it decreases injury to the client and the consequent risk of lawsuits. Computers increase efficiency and reduce cost as a result. Lawsuits do not increase because of computer use; they increase when computers are used improperly.

Answer to Question 2

2. No clear timetables exist, nor are there clear-cut stages of grief.

Rationale:
Although the models of grief are useful in guiding nursing care of clients who are experiencing loss, there are no clear-cut stages of grief, nor are there exact timetables. Kbler-Ross describes all stages of grief and grieving. None of the models are clear or definitive predictors of grief behaviors. No research exists that proves these models are not useful.

There can actually be a 25-hour time difference between certain locations in the world. The International Date Line passes between the islands of Samoa and American Samoa. It is not a straight line, but "zig-zags" around various island chains. Therefore, Samoa and nearby islands have one date, while American Samoa and nearby islands are one day behind. Daylight saving time is used in some islands, but not in others—further shifting the hours out of sync with natural time.

Drug abusers experience the following scenario: The pleasure given by their drug (or drugs) of choice is so strong that it is difficult to eradicate even after years of staying away from the substances involved. Certain triggers may cause a drug abuser to relapse. Research shows that long-term drug abuse results in significant changes in brain function that persist long after an individual stops using drugs. It is most important to realize that the same is true of not just illegal substances but alcohol and tobacco as well.
