The client in CRF with significant osteodystrophy (osteoporosis, or calcium loss from the bones) is at high risk for fractures; therefore, preventing injury is the most appropriate outcome. The client is at risk for anemia, but not bleeding. The client on hemodialysis may have a disturbed body image, but in this case, it is specified that the client has significant osteodystrophy. Anxiety is not related to osteodystrophy.

2. Ask management for the use of a conference room.

Rationale:
To comply with HIPAA, nurses cannot discuss events involving clients in any setting where the conversation can be heard by others, so a private conference room would be the best solution. The nurses must also guard against other health professionals not directly involved with the client from overhearing their discussion; consequently, a staff elevator is not acceptable. Discussing the event outside the hospital is inappropriate because anyone could overhear the conversation. This would also preclude nurses from discussing client care in the home.
