Answer to Question 1

A
Narrative charting, the traditional method of nursing documentation, is a story format that describes the patient's status, interventions, treatments, and response to treatments. Focus charting organizes narrative documentation into data, action, and response. Charting by exception requires the nurse to document only deviations from preestablished organizational norms. Problem-oriented charting uses the steps of SOAP or SOAPIER when documenting.

Answer to Question 2
