Your client has yellow drainage from a wound. What does this fact indicate?
 
  1. The wound is infected.
  2. The client has a stage III ulcer.
  3. The client has a stage IV ulcer.
  4. It is unclear whether this wound is infected.

You are caring for a client who experiences the protrusion of an internal organ (bowel loop) through an incision. You should do which of the following first?
 
  1. This is an emergency. Contact the physician immediately.
  2. Cover the protruding intestine with a sterile dressing moistened with sterile normal saline solution to help prevent wound contamination, and keep the abdominal contents moist.
  3. Monitor the client closely and assess vital signs and pulse oximetry readings. If the client goes into shock, contact the physician.
  4. Provide emotional support to the client, because the event is extremely frightening.
