Answer to Question 1

D
Clear soft tapping indicates the systolic sound or the first phase of the blood pressure. The second phase is the swishing sound. The muffled sound is the fourth phase and in children or physically active adults, is considered the diastolic reading. The best indication of diastolic pressure, however, is when the sound disappears as in phase five of the blood pressure reading.

Answer to Question 2

B
The metaparadigm of medicine focuses on the pathophysiology and curing of disease. Nursing's metaparadigm is broader and focuses on the person, health, and the environment.
