Answer to Question 1

A
When a nurse assesses the blood pressure using the brachial artery, the arm should be positioned at heart level with the elbow extended and palm turned upward. The cuff should be inflated to 30 mm Hg above the diminished pulse point. The diaphragm chest piece should be placed after the cuff is inflated. The nurse should wait 2 minutes before repeating the blood pressure assessment.

Answer to Question 2

A
Costal breathing occurs when the external intercostal muscles and the other accessory muscles are used to move the chest upward and outward. Diaphragmatic breathing occurs when the diaphragm contracts and relaxes as observed by movement of the abdomen. Dyspnea refers to difficulty in breathing as observed by labored or forced respirations through the use of accessory muscles in the chest and neck to breathe. Cyanosis is a bluish color that results from reduced oxygen levels in the arterial blood.
