The nurse should monitor the client for signs of fluid and electrolyte imbalance; encourage the client to eat foods low in residue if taking a chemotherapy drug known to cause diarrhea; and urge the client to avoid foods, such as warm liquids, that stimulate the gastrointestinal tract. The client should be monitored for any signs of skin breakdown, and the perineum should be cleansed and dried after each loose stool. Sitz baths may be prescribed for client comfort and antidiarrheal medications may also be prescribed.
