Answer to Question 1

B
Constipation is a common problem in clients who are elderly. The nurse can encourage adequate fiber intake, increased fluids, regular exercise, and a regular schedule to improve the condition.

Answer to Question 2

A
Pyloric stenosis occurs when the circular muscle surrounding the pylorus blocks the gastric emptying. The infant will have projectile vomiting, ravenous hunger, hyperactive bowel sounds, irritability, decreased number and volume of stools, an olive-shaped mass in the right upper quadrant, and visible peristaltic waves. Surgery is required to correct the problem and usually has no lasting effects.
