Answer to Question 1

C
Feedback:
Clients with HIV have decreased immunity and are prone to infections. Infection in a client with HIV is life-threatening, because it makes the client vulnerable to other infections, and also impairs his or her already weakened immune functions. Clients with HIV may not have problems with other activities and food. They may often feel depressed, but this is not the highest priority.

Answer to Question 2

C
Feedback:
The nurse can prevent oxygen toxicity from occurring in a client on HBOT by providing the client with brief periods of breathing room air. HBOT consists of the delivery of 100 oxygen at three times the normal atmospheric pressure within an airtight chamber. Treatments, which last 90 minutes are repeated over days, weeks, or months of therapy. Reducing the amount of time oxygen is delivered over the atmospheric pressure, reducing the treatment time, or performing the therapy once in 2 weeks would not help to prevent oxygen toxicity in a client.

Though newer “smart” infusion pumps are increasingly becoming more sophisticated, they cannot prevent all programming and administration errors. Health care professionals that use smart infusion pumps must still practice the rights of medication administration and have other professionals double-check all high-risk infusions.
