Answer to Question 1

C
An intensive care unit is the most expensive delivery site for medical care because each nurse is usually assigned to care for only one or two patients at a time and because of the types of treat-ments and procedures the patients in the intensive care unit typically require. Rehabilitation units, long-term care facility, and a private hospital room are not as expensive per day as an intensive care unit.

Answer to Question 2

C
The 64-year-old patient with heart failure has a chronic disease and more risks than the other pa-tients because of age and income. Some patients are more in need of discharge planning because of their risks. For example, some patients have limited financial resources or limited family sup-port; others may have long-term disabilities or chronic illnesses. Early discharge teaching is es-pecially important as a way to decrease readmission to the hospital. Appendectomy and hyste-rectomy are acute conditions and do not have as many risks as heart failure. A healthy newborn is stable, whereas heart failure is chronic.
