Informed consent of a minor is required by the parent or guardian with the exception of when a minor lives on his or her own; is married; becomes pregnant; requires treatment for sexually transmitted diseases, substance abuse, or mental illness; or is in an emergency situation when parent or guardians are unable to be reach and there is a life-threatening situation.

For certain medical or surgical treatments or procedures, the client's consent is obtained in writing on a form; the physician who will perform the procedure is responsible for explaining the procedure and its risks and benefits to the client and for obtaining the client's signature. The client may turn to the nurse, however, for additional information. If the client does not appear to understand what the procedure involves, the nurse should document this in the client's record and inform the physician. A signature on a form does not always indicate a client's understanding or willingness to consent. The nurse should always be aware of any indications that the client has been coerced into signing the form against his or her wishes.
