Rationale: The discoloration of the teeth and metabolic alkalosis in a healthy adolescent would be highly suspicious of possible purging-type bulimia nervosa. Exercise level is difficult to determine because even normal activity levels can build well defined muscles. Anorexia nervosa would not explain the findings of teeth discoloration.
