Answer to Question 1

4
Rationale: The client is exhibiting all the signs of hypothermia. Vital signs are all decreased, so the client is not experiencing pyrexia (fever), or a remittent fever, which would occur with an elevated temperature. There is no such thing as vital sign crisis.

Answer to Question 2

3
Rationale: Needleless systems and needle safety devices are used to protect the safety of healthcare personnel
