A client who was widowed 18 months ago is seen in the internal medicine office. She mentions that she started crying when she came across a picture of her late husband. An appropriate response for the nurse would be:

  1. It seems to be a long time for you to still be crying. Have you considered counseling?
  2. It is very normal for anniversaries, pictures, and holidays to trigger feelings of loss.
  3. Do you have a pet? Pets often can help people move on from a loss.
  4. Are you keeping busy? You need to develop new interests to keep from crying.

Question 2

The LPN/LVN can have several responsibilities regarding the client's assessment, and it is most important to know:

  1. The difference between a complete and focused assessment.
  2. The scope of practice defined by the state board of nursing.
  3. Who can delegate to the LPN/LVN.
  4. The sequence of assessment.
