Have you ever felt this depressed before? is a closed-ended question requiring only a yes or no response and so provides minimal information regarding the client's condition.
While the question What makes you feel that you are experiencing depression? is an open-ended question, it is not the best option because it is not directed toward assessment of the client's current complaint.
While the question What can we do to make you comfortable while you are here? is an open-ended question, it is not the best option because it is directed at the client's comfort, not toward assessing his or her current complaint.

Intravenous pyelography is performed by administering iodine-based dye to view functionality of the urinary system. Many individuals are allergic to shellfish; therefore, the first nursing priority is to assess the patient for an allergic reaction that could be life threatening. The nurse should then encourage the patient to drink fluids to flush dye resulting from the procedure. Narcotics can be administered but are not the first priority. Turning the patient on the side will not affect patient safety.
