Answer to Question 1

A
The nurse should encourage clients to avoid drinking or ingesting caffeine before bedtime. Cof-fee, tea, and chocolate act as stimulants, and can cause a person to stay awake or awaken throughout the night. Coffee, tea, colas, and alcohol act as diuretics and cause a person to awak-en and void at night.
A vegetarian diet would not be causing sleeplessness.
An afternoon exercise program would not be causing sleeplessness.
A hot bath in the evening may promote relaxation and would not cause sleeplessness.

Answer to Question 2

A Washing hands before applying a dressing is a correct action to help prevent a nosocomial infection.
B Taping a plastic bag to the bed rail for tissue disposal is a correct action to aid the patient in proper disposal of secretions.
C The staff member who places the Foley catheter bag on the bed when transferring the patient is placing the patient at risk for a nosocomial infection because urine in the catheter or drainage tube may re-enter the bladder (reflux).
