Herbs are a common method of treatment in many cultures. If a client traditionally drinks an herbal tea to alleviate symptoms of an illness, there is no reason why both the herbal tea and the prescribed medications cannot be used as long as the tea is safe to drink and does not interfere with, or exaggerate, the action of the medications. Asking why the patient would want to drink that stuff is demeaning to the patient. Answer d is incorrect because there is no paperwork necessary.
