With the case management model of care, the case manager coordinates the efforts of all disci-plines to achieve the most efficient and appropriate plan of care. Continuity of care is of primary importance.
If the efforts of all disciplines are well managed, repetition or delays may be avoided with a re-sultant shortened hospital stay. Therefore this focus of care may not be more expensive.
The physician may or may not be the coordinator of client care. The case manager typically is a nurse or social worker.
Case management is not entirely based on the level of care required.
