Answer to Question 1

A
A. Distraction measures, such as turning on a sink faucet so the client can hear water running, may help the client to void.
B. Assist the client into a normal voiding position (standing for a man, squatting for a woman). One factor that can interfere with urination/micturition is bed rest or immobility, which does not allow the client to assume the normal position for emptying the bladder.
C. When bladder is full, palpation causes urge; palpation causes pain.
D. Clients taking diuretic medications should receive them early in the morning so they do not need to void during the night.
