Answer to Question 1

When planning a dive operation, a comprehensive safety and health assessment must be conducted. The following areas should be assessed: 1) diving mode; 2) conditions on the surface and underwater; 3) hazards on the surface and underwater; 4) supply of breathing gas; 5) supply of breathing gas reserves; 6) thermal protection; 7) diving equipment, tools, and systems; 8) Physical fitness of all dive team members (fitness level appropriate to the assignment, physical impairments from injuries or illnesses, etc.); 9) assignments for each member of the dive team; 10) repetitive dive designation or residual inert gas status for all dive members; 11) decompression and treatment procedures; and 12) applicable emergency procedures.

Answer to Question 2

The company's safe-diving procedures manual should contain at least the following information beyond the actual OSHA standard (29 CFR 1926, Subpart Y): 1) safety procedures and corresponding checklists for diving operations; 2) assignments and responsibilities of all dive team members; 3) equipment procedures and corresponding checklists; and 4) emergency procedures for all predictable problems including fire, equipment failure, medical problems and illnesses, injuries, and hazardous environmental conditions.
