Answer to Question 1

The stress of surgery will cause a change in BEE, insulin utilization, lipid utilization, and possible fluid shifts. This would make it impractical, if not impossible, to monitor and control TPN during surgery. However, if TPN were stopped completely without providing a source of glucose, the blood glucose level could drop very low; therefore, D10W was hung.

Answer to Question 2

BEE = 66 + 13.7  wt + 5  ht - 6.8  age
= 66 + 13.7  76.4 + 5  177.8  6.8  58
= 66 + 1047 + 889  394
= 2029  394
= 1635

Using a stress factor of 1.4: 1635  1.4 = 2289
