Obtain a guideline for the Nutrition Screening Initiative (available from: ADA, see http://www.eatright.org/Public/SearchResults.cfm). Using this guide, identify the agencies that should be contacted for each of the problems in the outline above. With the Nutrition Screening Initiative as a guide, discuss possible solutions to the problems mentioned above.

The Nutrition Screening Initiative1,2 has a list of services and agencies available for serving the elderly. Addresses and phone numbers are also listed for agencies (as the State Agency on Aging) in various states where available. Checklists are included to help the allied health worker to identify the service most suitable for the problem in question, such as social services, mental health, State Agency on Aging, nutrition counseling, medication use, etc. Each of the agencies mentioned above could be contacted for Mrs. SJ.

Answer to Question 2

Because of her gastritis, a diet low in caffeine would be in order. This could also help with dizziness, arrhythmias, and insomnia. A moderate- to low-sodium diet due to her edema and heart problems would be recommended, along with the usual healthy diet with low total fat, low saturated fat, low cholesterol, adequate calcium, iron, fiber, vitamins C, A, and D, etc. Instruction on healthy snacks would be good so she will know what to do when she feels weak.
